The ACCP Updates in Therapeutics®: Patient-Centered Team-Based Practice Forum offers a series of hands-on, interactive programs to assist practitioners with developing and/or enhancing their clinical pharmacy services to deliver comprehensive medication management. Attendees will get a first look at the learnings of the CMM Effectiveness and Implementation Grant study, which will be deployed for use during this engaging forum.

This live forum will be delivered using various active learning strategies through plenaries and workshops, and will feature a keynote from the Centers for Medicare & Medicaid Services.

The Patient-Centered Team-Based Practice Forum will be held February 16-18, 2018, in Jacksonville, Florida.
